Determination of TSH and free thyroid hormones in the diagnosis of feline hyperthyroidism

Abstract
In 59 euthyroid and 10 hyperthyroid cats serum activity of TSH and serum concentrations of total (TT3, TT4) and free thyroid hormons (FT,, FT,) were determined using chemiluminescent technology (CLA) designed for human sera (ACS:180, Bayer Vital). Additionally a second CLA for human TSH was tested in eight cats (Immulite, DPC Biermann). Diagnosis of hyperthyroidism was confirmed by clinical examination, laboratory results and in seven cases by demonstration of toxic nodular goiter by thyroid scintigraphy. Determination of feline TSH was not possible using the Immulite assay. CLA results using the ACS:180 were validated by calculation of intraassay precision using pooled sera of 27 euthyroid cats. Variation coefficients for FT3, FT4 and TSH were 6.3%, 4.8%, and 2.7%, respectively. Total thyroid hormone determination showed somewhat higher variation coefficients of 10.2% and 39.0%, but was still acceptable. Additionally, intraassay precision for TSH was determined with sera of five cats in different functional thyroid situation. Variation coefficients were 4.3 +/- 1.5%. In healthy cats, median TSH was 0.24 mU/l with a deviation of 0.20 mU/l. All results obtained using the ACS:180 were significantly different in patients and controls (p <0.001, TT4: p <0.005). Diagnosis of feline hyperthyroidism is reliably possible by determination of serum TSH activity using a anti human TSH chemiluminometric immunoassay.
