Abstract:
While being fully aware of the fact that in healthy persons there may be circadian variations of physiological and psychophysiological functions as well as disturbances of the basal levels and in the diurnal behaviour of endocrinological parameters in rheumatic diseases, we determined in 22 patients with confirmed ankylosing spondylitis and in a comparable healthy control group the diurnal rhythm of osteocalcine, bone-specific alkaline phosphatase and cortisol (as parameters with known influence exercised on bone metabolism). We were able to confirm for these parameters a circadian rhythm without indication of any desynchronisation. In men suffering from ankylosing spondylitis there was a tendency to levels of higher concentration of osteocalcine and of bone-specific alkaline phosphatase. However, whether this is indicative of an enhanced bone remodelling will have to be verified by future studies. The physiological circadian rhythm of cortisol secretion in ankylosing spondylitis is possibly responsible for the poor therapeutic efficacy of a systemic corticosteroid dosage on the clinical pattern of symptoms. When taking samples during clinical routine attention should always be paid to the circadian rhythm of the investigated parameters to avoid misinterpretation of results.
