EFFECTS OF NUTRITIONAL SUPPLEMENTATION DURING PREGNANCY UPON BIRTH-WEIGHT - THE INFLUENCE OF PRE-SUPPLEMENTATION DIET

Abstract
Poor urban families (456) at risk of malnutrition in which the mother was in the 1st or 2nd trimester of pregnancy were enrolled in a food supplementation study. After an initial dietary survey 230 families were randomly selected as controls. Supplements were provided to the remaining 226 families in amounts sufficient to meet a substantial proportion of the recommended dietary allowances for pregnant mothers and all other members of the family. Treatment effects on mean birth weight were previously reported. The influence of dietary intake prior to supplementation upon the effect of supplementation on maternal diet and birth weight is reported here. The control and supplemented groups were divided into 2 subsets according to initial dietary intake: those consuming > 1500 calories or 30 g of protein and those consuming < 1500 calories or 30 g of protein prior to supplementation. Protein intake increased in both subsets while calorie intake increased significantly only in the initial low intake group. Birth weight increments as a result of supplementation were found only among the initial high intake subset. Maternal weight gain increased in both groups. The supplementation program was insufficient to affect the birth weight among the half of the sample initially most severely deprived of calories and proteins. These women may have been nutritionally deprived to such a degree that more substantial improvement of their nutritional status would have been necessary to increase offspring birth weight.
