Abstract
Extracorporeal life support (ECLS) is frequently used to stabilize patients with acute circulatory failure as abridge to recovery or to definitive treatment. Despite its increasing use, ECLS is still associated with consistently high mortality rates around 50%. Previous in vitro and in silico studies demonstrated that with conventional cannulation strategies for ECLS the perfusion of the coronary arteries and the supra-aortic vessels is insufficient. Furthermore, ECLS increases the left ventricular afterload and impairs left ventricular unloading. These aspects impair patient recovery. The presented project, which is supported by the Dr. Rusche research project 2019, aims at developing anew double lumen cannula. This cannula is intended to be inserted via the left ventricular apex. The cannula tip is placed in the aortic root and allows antegrade perfusion of the whole body. The left ventricle is vented over the second lumen of the cannula by active suction. This principle addresses the limitations of conventional ECLS cannulation strategies and could improve cardiac recovery after ECLS. The project includes the optimization of the cannula design using computational fluid dynamics simulation, the production of prototypes as well as testing of these prototypes in large animal studies. In the long run, the cannula is intended to be brought into clinical application.
